CRE enrollment form

All new and returning children and youth must be registered after three visits in order to attend classes or childcare on Sunday mornings.

The enrollment form must be filled out for each family enrolling a child and includes one copy of the medical form.

If you plan on visiting more than three times throughout the year, we ask that you complete the RE Enrollment Form.

Why enroll?

• We want to keep your child happy and need a way to contact you if necessary.
• We want to keep your child healthy and need to know if there are any medical concerns that we need to be aware of.
• We need to keep your child safe and want to release them to only the adults you designate.
